Hospice Support Services
Consider us your support team encompassing not only the needs of the patient but the family members as well.
At Omni Care Hospice, we are sensitive to the needs of our patients and their families. Increasing the quality of life for an individual with a life-limiting condition can be stressful and requires support outside of just the medical field.
That’s why we offer hospice support services to each individual we work with and their loved ones. We want to be your support system.
If you have questions regarding hospice support services, contact a Care Representative at (702) 509-5276.
Our social workers can help communicate the patient’s needs to their team of medical personnel or provide assistance with filling out insurance forms and addressing any insurance issues that come up.
Hospice Isn’t Just for the Patient!
Here at Omni Care, we understand that this is a time of transition for everyone involved, and it may be emotional or stressful at times. That’s why our philosophy is to work with all who are involved and provide as much support as we can.
We’re all in this together! As a general rule, hospice social workers follow the wishes and requests of the family. They are available to offer a more holistic approach during this end-of-life transition that encompasses physical and emotional needs not only of the patient but also for the family.
Patient & Family Education
Our social workers are here to help patients and their loved ones understand what they can expect from the hospice process. They are available to help answer any questions you might have and suggest additional resources if necessary; This can include what is needed in regards to patient care from nutrition and dietary tips to physical care such as bathing.
Assistance With Final Details
Our social workers can help with things such as funeral arrangements and are available to provide direction to resources that can help with legal documents such as advanced directives, DNRs or wills. They are also available to suggest community resources to assist families as they go through this difficult transition.
The hospice care social worker is available as a resource to connect the family with grief counseling as well as other after-death arrangements that have not been taken care of or thought of before the passing of the loved one. You can count on us to be there every step of the way.
