The common home hospice answers that your healthcare office is looking for.
If a referral is called in we will do a check on eligibility for Medicare immediately. Once approved we will send an admissions nurse to do a start of care (SOC) on the patient within 24 hours.
We take ALL Medicare patients that are eligible.
We can have the referral forms faxed to us and also utilize to option for a staff member of Omni Care Hospice (OCH) to pick up the referrals.
Hospice is considered a 24-hour service that works 365 days.
- Order for Hospice Services written by the physician
- Face Sheet with all the patient’s demographics
- Medicare number
- History and Physical
- Medication list
Upon, receiving a patient referral and all is approved we will have our admissions nurse do the evaluation and admit within 24 hrs of receiving the referral. Typically, we see the patient within 2-4 hours after getting the referral but say 24 hours to be on safe side.
Community liaisons are credentialed to enter multiple hospitals and health care facilities throughout Las Vegas.
Once the Case Manager assesses the patient’s pain level and /or infection the CM then relays all the information to our medical director. After that, our medical director will determine what type of medication is appropriate for that patient.
In hospice, a DNR (Do Not Resuscitate) does not have to be in place. We are obliged, though, to try and resuscitate the patient. If a patient does have a DNR in place, we will let nature take its course and let the patient pass peacefully.
Service is provided in the Las Vegas metro area and Boulder City. We have a 50-mile radius from our headquarters.
Yes, we provide care for our patients 24 hours a day, 7 days a week. 365 days a year.
Registered & Licensed Nurses
Assess patients and provide care that the physician has ordered. This includes performing prescribed medications, assessing a chronic illness in which the patient’s condition has deteriorated; and teaching treatment, health measured and disease processes.
Our Therapists will assess and utilize rehabilitative techniques of exercise, gait training, prosthetics and heat to restore patients to their highest functional level of strength, range of motion and mobility.
Medical Social Services
Our Social Workers will provide counseling about social or financial difficulties arising from a patient’s medical condition. We will also provide access to community resources to assist the patient with their needs.
